Job summary

Springwell lives by helping older adults age-in-place and support individuals with disabilities in the community. A Case Management role offers in-home visits, comprehensive assessments, and collaboration with an interdisciplinary team to craft person-centered care plans.

The role requires a Bachelor's or higher, or an Associate's with relevant elder care experience, plus strong organizational skills and data-reporting ability. Reliable transportation is required.

Qualifications

• Bachelor's Degree or higher required; or Associate's with 3+ years elder/case management experience.
• Willing to complete remaining Bachelor's coursework within 3 years if needed.
• Strong desire to work with seniors and individuals with disabilities.
• Excellent organizational skills and adaptability to change.
• Experience with computers, data entry, and generating reports.
• Reliable car, valid driver's license, and safe driving record.

Job description

Are you seeking a Case Management position? Come work for Springwell and help older adults age-in-place and adults with disabilities receive care in the community.

The compensation for this role is $28.21/hr for a 37.5 hour work-week.

DESCRIPTION:

• Work collaboratively as part of an interdisciplinary team that provides in-home services to older adults and individuals with disabilities.
• Make home visits, conduct comprehensive assessments, and participate in the development of person-centered care plans.

QUALIFICATIONS:

• Bachelor's Degree or higher.
• Applicants without a Bachelor's Degree may be considered if:
  • The applicant has an Associate's Degree and at least 3 years' experience working with elders or providing case management services; OR
  • The applicant has completed at least 50% of coursework required for a Bachelor's Degree and agrees to complete the remaining coursework within a 3-year period.
• A strong desire to work with seniors and individuals with disabilities.
• Excellent organizational skills and the ability to manage constant change with a positive attitude.
• Experience working with computers, entering data, and running reports.
• Applicant must have a reliable car, current driver's license, and safe driving record.
