Job summary

Team Recovery in Toledo, OH seeks an Intake & Assessment Therapist to conduct clinical assessments, identify treatment needs, and help individuals access the appropriate level of care. You'll collaborate with medical, clinical, and administrative staff to support safe transitions into treatment.

This full-time, exempt role involves three 12-hour shifts weekly with one weekend per month. Bachelor's degree and LSW license required, plus strong documentation and teamwork skills.

Qualifications

• Bachelor's degree in social work, behavioral science or related field and licensure as applicable.
• Current Licensed Social Worker (LSW) license.
• Ability to complete clinical documentation accurately and timely.
• Knowledge of behavioral health and substance use disorders and related care needs.
• Strong communication, organization, time-management, and problem-solving skills.
• Ability to work with diverse populations and maintain safety.

Responsibilities

• Conduct clinical and diagnostic assessments to evaluate concerns and determine care level.
• Develop and initiate individualized treatment plans based on assessments.
• Complete accurate, timely clinical documentation for services.
• Identify safety concerns and assist with safety planning as needed.
• Respond to crises and coordinate with emergency personnel when necessary.
• Collaborate with clinical, medical, and administrative teams to facilitate admissions.
• Maintain a safe, supportive environment for clients and staff.
• Participate in trainings, meetings, and company initiatives.
• Perform related duties to support the treatment team.
