Team Recovery LLC in Toledo, OH seeks an Intake & Assessment Therapist to conduct clinical assessments, develop treatment plans, and help clients access appropriate levels of care. You will support admissions and coordinate with clinical and administrative staff.

This role requires licensure (LSW/LPC/LCDC), strong communication, and ability to work in a fast-paced setting. CPR/BLS certification is required or obtained after hire; schedule is three 12-hour shifts with one weekend per month.

Qualifications

• Bachelor's degree in social work or related field.
• Active LSW/LPC/LCDC license.
• Knowledge of behavioral health and substance use disorders.
• Strong communication and organizational skills.
• Ability to work with diverse backgrounds.
• Valid driver's license and insurability.

Responsabilités

• Conduct clinical and diagnostic assessments to evaluate concerns and determine levels of care.
• Develop and initiate individualized treatment plans based on assessments.
• Complete accurate, timely clinical documentation for all services.
• Identify safety concerns, conduct risk assessments, and assist with safety planning.
• Respond to crises, support de-escalation, and coordinate with emergency personnel as needed.
• Collaborate with clinical, medical, and administrative teams to facilitate admissions.
• Maintain a safe, supportive environment for clients and staff.
• Participate in training and team meetings.
• Perform other duties to support the treatment team.
