Qualifications

• Bachelor's degree (LSW or RN preferred), or Master's degree in social work or counseling with licensure.

Responsabilités

• Coordinate Clinical Care: Work with interdisciplinary teams to update clinical information.
• Drive Treatment Planning: Lead admissions with psychosocial assessments and recovery roadmaps.
• Connect the Community: Provide resource information and warm referrals to providers.
• Manage Admissions: Handle intake, verify benefits and obtain necessary authorizations.
