Job summary

CLASS (Community Living And Support Services) in Pittsburgh seeks an Intake Coordinator to welcome, register and admit participants into our services. You will gather records, assess needs and coordinate with participants, families and funders to move referrals forward.

The role requires collaboration with program staff, familiarity with EHR/Therap systems, and strong communication. A Bachelor’s in Human Services or related field is preferred, with experience in admissions, case management or

Qualifications

Bachelor’s Degree in Human Services or related field preferred; or two years of experience with people with disabilities.
Admissions and case management experience preferred.
HHAX, EHR, and Therap software experience preferred.
Working knowledge of Outlook, Word and Excel.

Responsibilities

Searches for and recommends viable new referrals to the agency by working with program directors.
Link referral candidates to the program of choice and guide participants/families in accessing funding.
Develop plans and conduct placement follow-up meetings with program directors.
Communicate with Service Coordinators monthly about openings and status.
Coordinate intake visits in participants' homes and complete intake assessments.
Maintain documentation in EHR/Therap for service delivery notes and billing.
