Responsibilities

• Intake & Admissions: conduct eligibility assessments and complete intake documents.
• Documentation & Compliance: maintain confidential records and coordinate releases of information.
• Beneficiary Support: assist with medication, insurance, and healthcare needs; support Medicaid enrollment.
• Community Outreach & Partnerships: build relationships with community organizations and coordinate outreach events.
• Data & Reporting: track admissions, census, and ensure data accuracy.
• Additional Responsibilities: conduct intake testing and maintain office supplies.
