Job summary

Hartford Hospital is seeking a Full Time Wound Care/Ostomy Registered Nurse for the Inpatient WOCN Team in Hartford, CT. The role provides expert wound, ostomy and continence care, educating patients and families while coordinating transitions across the care continuum.

The ideal candidate holds a BSN, with WOCNCB certification preferred, and has recent acute care experience. Weekend coverage is required, and the position reports to the Inpatient Wound Team program manager.

Qualifications

• BSN required.
• WOCNCB accredited education with current certification preferred.
• Weekend coverage required; exceptions may be considered.
• Recent acute care experience preferred.

Responsibilities

• Provide direct care to patients with complex wounds, ostomy care and continence issues.
• Collaborate with health care team to develop and implement care plans.
• Educate patients, families and staff on wound/ostomy care and self-management.
• Evaluate patient response to treatment and adjust care plans as needed.
• Use evidence-based guidelines to prevent and treat skin/wound conditions.
• Participate in quality improvement, training and data analysis.
• Document care accurately in the electronic medical record.
• Join the Inpatient WOCN Team at Hartford Hospital.
