Job summary

Hartford Hospital is seeking a Wound Care/Ostomy Registered Nurse to provide expert wound, ostomy and continence care within the Inpatient WOCN Team. The role requires utilizing the nursing process for prevention and treatment across all patient populations.

The position emphasizes direct patient care, staff education, and collaboration with multidisciplinary teams, reporting to the Program Manager, Inpatient Wound Team.

Qualifications

• BSN required.
• Graduate of a WOCNCB accredited education program with current Wound/Ostomy/Continence certification preferred.
• Recent acute care experience preferred. Weekend coverage required.

Responsibilities

• Provides direct care to patients with complex acute and chronic wounds, ostomy care and continence needs.
• Collaborates with the health care team to develop and implement plans for wound and ostomy management.
• Educates patients, families, caregivers and staff on wound, ostomy and continence care.
• Evaluates patient response to treatment and updates care plans as needed.
• Uses Evidence-based guidelines and documents care in the EMR.
• Participates in orientation, training, quality improvement activities and policy development.
• Joins the Inpatient WOCN Team at Hartford Hospital.
