Job summary

Hartford Hospital is seeking a Full Time Wound Care/Ostomy Registered Nurse to provide expert nursing consultation for patients with acute and chronic wounds, ostomy and continence care. The role includes direct patient care, education, and collaboration with the Inpatient WOCN Team at Hartford Hospital.

The candidate will apply the nursing process within our professional practice framework, support transitions across care settings, and contribute to quality improvement and policy development.

Qualifications

• BSN required
• WOCNCB accredited education program with current wound/ostomy/continence certification preferred
• Weekend coverage may be required

Responsibilities

• Provides direct care to patients with complex wounds, ostomy care and incontinence management.
• Collaborates with health care team to develop and implement care plans for skin integrity and ostomy care.
• Educates patients, families, and staff on wound and ostomy care, and percutaneous tubes.
• Evaluates patient response and adjusts care plans to improve outcomes.
• Uses protocols and evidence-based guidelines for wound care and ostomy management.
• Participates in orientation, training, quality improvement, and data analytics activities.
• Documents care accurately in the electronic medical record.
• Collaborates with Inpatient WOCN Team at Hartford Hospital.
