Responsibilities

• Completes wound assessment and develops appropriate treatment and plan of care based on patient and family needs.
• Implements plan of care, nursing interventions, and patient care procedures.
• Modifies plan of care based on continuous evaluation.
• Maintains up-to-date documentation of nursing care for information sharing across the care team.
• Provides consultation for complex wounds, pressure injuries, surgical wounds, and skin integrity concerns.
• Collaborates with physicians, advanced practice providers, bedside nurses, rehabilitation, case management, nutrition, and other disciplines to optimize outcomes.
• Inputs charges and documentation for wound care patients into the computer system in a timely manner.
