Oregon Health & Science University is seeking a Registered Nurse Liaison based at OHSU Hospital with travel to Portland metro area sites. You will conduct clinical assessments, coordinate discharge planning, and support patient education for complex home infusion regimens.

The role emphasizes interdisciplinary collaboration, familiarity with Epic, and the ability to work in a fast-paced acute care environment while supporting safe transitions to home infusion services.

Qualifications

• Advanced clinical assessment skills for transition readiness and home safety.
• Strong interdisciplinary communication, patient education, and advocacy skills.
• Proficiency with Electronic Health Records (Epic) and Microsoft Office.
• Ability to work independently in a fast-paced acute care environment.

Responsabilités

• Conduct in-person clinical assessments and evaluate therapy appropriateness.
• Assess caregiver competency and deliver patient education.
• Coordinate multi-disciplinary discharge plans across teams including physicians, APPs, pharmacists, case managers, social workers.
• Travel between OHSU campuses and affiliated sites as needed; work with home infusion programs.
