Job summary

Oregon Health & Science University is seeking a licensed pharmacist to provide safe and proper distribution and use of medications in outpatient home infusion settings. The role includes independent decision-making, collaboration with healthcare teams, and supervision of students and residents.

Relief-STAFF position with availability to work at least four days per pay period, and on-call weekdays and weekends. Oregon license required; ambulatory infusion experience is preferred.

Qualifications

• Active pharmacist license in Oregon.
• Time of recruitment may require specialty training as determined by OHSU.

Responsibilities

• Identify, resolve and document complex therapeutic problems for home infusion patients.
• Respond accurately to drug information requests from physicians, nurses, patients and families; review literature for patient care.
• Monitor therapeutic response and adverse effects; report adverse drug events and medication errors.
• Direct the work of students/residents and evaluate their performance.
• Influence therapy standards by collaborating with physicians and health-care teams.
