Job summary

Responsibilities

• Facilitates identification and resolution of gaps in care from hospitalization, at discharge, and into ambulatory care.
• Leads development and implementation of complex plans of care with defined goals in collaboration with the interprofessional team.
• Champions evidence-based practice and supports new standards of care.
• Evaluates patient and family experience and improves satisfaction and quality of care.
