Job summary

Qualifications

• Under the direction of the manager, plans, directs, and coordinates activities of a unit on a designated shift.
• Performs direct patient care as needed and serves as a clinical resource to nursing and support staff.
• Assesses, plans, evaluates and documents the nursing process of the assigned unit.

Responsibilities

• Facilitates identification and resolution of gaps in care from hospitalization to discharge and ambulatory care.
• Leads development and implementation of complex care plans with goals in collaboration with interprofessional team, patient, family, and caregiver.
• Champions evidence-based practices and supports practice change based on new knowledge.
• Analyzes gaps in practice standards and provides feedback to peers to enhance safe patient care.
• Safety-sensitive position requiring constant alertness.
