Job summary

Cleveland Clinic Health System is seeking a Clinical Pharmacist for inpatient, surgical services at the Main Campus in Cleveland. The role covers central operations and surgical units such as orthopedics and trauma stepdown, with daily responsibilities including medication safety, pharmacotherapy planning, and clinical coverage.

The candidate will precept students and residents, engage in departmental projects, and support medication reconciliation.

Qualifications

• Pharmacist degree required (PharmD/BS Pharm).
• Active state licensure or reciprocation within 90 days of hire.
• Immunization administration/safety certifications (BLS/ACLS/PALS) required or within 90 days.

Responsibilities

• Provide patient-centered, evidence-based pharmacotherapy services.
• Coordinate pharmaceutical care for central operations and surgical units.
• Monitor therapeutic drug management and mitigate medication safety issues.
• Develop cost-effective pharmacotherapy plans and perform medication reconciliation.
• Precept APPE students and residents; participate in projects and committees.

Education

PharmD or Bachelor’s of Science in Pharmacy

Current State licensure as a Registered Pharmacist (RPh) or eligible for reciprocation within 90 days

BLS/ACLS/PALS certification requirements
