Qualifications

• Master's Degree in Speech-Language Pathology is required.
• AZ License with CCC-SLP or Clinical Fellow license is required.
• BLS certification is required before starting employment.

Responsibilities

• Implements individualized treatment plans for speech, language, cognitive-communication, voice and swallowing disorders.
• Evaluates patients under physician direction and develops measurable treatment goals and discharge plans.
• Maintains accurate and current documentation in medical records and billing per department procedures.
• Collaborates with interdisciplinary team to optimize discharge planning and patient outcomes.
• Participates in staff meetings, preceptor programs, and departmental initiatives.

Education

Master's Degree in Speech-Language Pathology from an accredited university approved by the ASHA

Arizona License to Practice Speech Language Pathology with CCC-SLP or Clinical Fellow with Temporary AZ License
