Job summary

Medical City Lewisville is seeking a Social Worker (BSW) PRN to support weekday and weekend shifts. The role focuses on discharge planning, community resource coordination, and patient-centered care within Med/Surg units. Training occurs on weekdays during the initial weeks.

The ideal candidate has a Bachelor’s in Social Work, TX licensure as a Registered BSW, and 1+ year of medical social work experience, with certification preferred. This is a flexible PRN position aligned with patient needs.

Qualifications

• Bachelor's Degree in Social Work required.
• Currently licensed as a Registered Bachelor of Social Work in the state TX.
• Certification in Social Work preferred.
• Minimum of 1+ years medical social work experience.

Responsibilities

• Works collaboratively with care team to identify and address patient needs and maintain effective communication.
• Identifies and addresses barriers to discharge and recovery (financial, emotional, psychosocial, medical, spiritual).
• Performs thorough assessments and develops discharge plans for high-risk patients with discharge planning or psychosocial needs.
• Obtains input from RN case manager to ensure discharge plan aligns with patient care plan and reassesses as needed.
• Participates in interdisciplinary care planning and proactive discharge planning.

Education

Bachelor's Degree in Social Work

Licensed as a Registered Bachelor of Social Work in TX

Certification in Social Work preferred
