HCA Florida UCF Lake Nona Hospital is hiring a Social Worker (MSW) to perform non-clinical biopsychosocial assessments and interventions across inpatient floors and the ED. You will engage in crisis intervention, discharge planning, and family-centered care while collaborating with a multidisciplinary team.

• MSW required.

• Master's degree in social work or related field.

Responsibilities

• Collaborate with RN case managers, nurses, physicians, insurance companies and staff to identify and address patient needs.
• Identify and address discharge barriers including financial, emotional, psychosocial, medical and spiritual aspects.
• Maintain knowledge of Medicare, Medicaid and managed-care regulations and length-of-stay considerations.
• Conduct thorough assessments and develop discharge plans for high-risk patients with psychosocial needs.
• Coordinate meetings between care team and patients/families/caregivers; support patient-centered decisions.
• Provide psychosocial support, counseling, advocacy and referrals to meet emotional, social and financial care needs.
