Job summary

Memorial Hermann Health System seeks a Licensed Master Social Worker to provide clinical social work and complex discharge planning for patients with psychosocial needs. Under supervision, you will assess, intervene, and coordinate services with an interdisciplinary team to ensure safe and timely transitions of care.

You will work with patients, families, physicians, and case managers to address emotional, financial, and social determinants affecting health outcomes, while guiding eligibility

Qualifications

• Graduate of an accredited Master of Social Work program (MSW).
• Current license as a Master Social Worker (LMSW) in Texas required; ACM certification from American Case Management Association (ACMA) preferred.
• Experience in health services/health care provider setting strongly preferred.
• Excellent communication skills and knowledge of DSM V and ICD-10.

Responsibilities

• Assess psychosocial risk factors and develop a treatment plan with the care team.
• Collaborate with physicians, case managers, and social work staff to support discharge planning.
• Provide crisis intervention and psychosocial assessments for patients and families.
• Support linkage to community resources and post-acute care options.
• Educate patients and families about treatment options and care planning.
