North Alabama Shoals Hospital is seeking a Social Worker to join our inpatient team. The role involves assessing patients and families, planning interventions, and coordinating discharge planning and community resources within a busy hospital setting.

The position requires a Master's degree and LMSW licensure, plus BLS-HCP certification and Crisis Intervention Training within 90 days of hire. Two years of related experience is preferred.

Formación

• Master's degree required.
• Licensed Master Social Worker (LMSW) License required.
• Basic Life Support Health Care Provider (BLS-HCP) certification.
• Crisis Intervention Training within 90 days of hire/transfer.
• 2 years related experience.

Responsabilidades

• Interviews and assesses patients and/or family, caregivers, and legal representatives.
• Prioritizes and arranges internal and external services/interventions.
• Participates in case reviews to evaluate management and progress.
• Consults with healthcare team to ensure treatment plan compliance.
• Assists with discharge planning and coordinates follow-up care and resources.
• Completes required documentation within required time frames.
