Job summary

North Alabama Shoals Hospital is seeking a PRN Social Worker for weekend coverage to support inpatient services. You will perform biopsychosocial assessments, plan interventions, and assist with discharge planning to ensure continuity of care.

The role requires a Master’s in Social Work and LMSW licensure, with BLS-HCP and Crisis Intervention Training preferred. You will work closely with the healthcare team to meet patient needs and community health goals.

Qualifications

• Master’s degree in social work (MSW) required.
• Licensed Master Social Worker (LMSW) licensed or eligible.
• BLS Health Care Provider (BLS-HCP) certification and Crisis Intervention Training within 90 days.
• Minimum 2 years of related social work experience.

Responsibilities

• Interviews and assesses patients and family, caregivers or legal representatives.
• Determines and arranges for needed internal and external services/interventions.
• Participates in case reviews to evaluate case management and progress.
• Assists with discharge planning and coordinates follow-up resources and referrals.
• Performs Medicaid screenings for the facility.
• Completes required documentation within required time frames.
