Job summary

North Alabama Medical Center in Florence, AL is seeking a Social Worker (LBSW) to join our case-management team. This full-time role focuses on helping patients cope with social, emotional, economic, and environmental challenges and coordinating services.

You will assess patients, participate in discharge planning, provide therapy, and collaborate with the health-care team. Qualifications include a Bachelor's degree, Alabama LBSW licensure within 6 months, and Handle With Care certification

Responsibilities

• Assists with discharge planning and identifies resources for housing, transportation and healthcare services.
• Supervises and trains new staff, students and interns.
• Provides individual, family and group therapy.

Social Worker Position Summary:
Assesses, and plans interventions to help patients cope with social, emotional, economic, and environmental problems.
Reports to: Director of Case Management
FLSA: Non-Exempt
Education: Bachelors required
Licenses: Board Certified Social Worker in the state of Alabama within 6 months of hire
Certifications: Handle With Care within 30 days of hire.

Required Skills
Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action.

Essential Functions
Interviews and assesses patients and/or patient's family, caregivers, and/or legal representatives.
Determines, prioritizes, provides and/or arranges for needed internal and external services/interventions.
Participates in case reviews to evaluate case management and progress. Consults with healthcare team members to promote, monitor, and evaluate compliance with patient's treatment plan.
Assists with discharge planning and processes. Identifies appropriate resources, including transportation, housing, healthcare, and social/spiritual services, and provides referrals as part of the discharge plan.
Supervises and/or trains new staff, students, and interns.
Provides individual, family and group therapy.
