Qualifications

• Master’s Degree in Social Work, counseling, or other human services related field.
• Required LCSW, LICSW, LMFT, or LHMC (or eligibility to receive one of these licensures in the future)
• Prior hospital social worker experience is preferred

Responsibilities

• Provide a comprehensive range of social work services to a variety of patients in an inpatient/partial hospital setting
• Act as the primary contact with external providers and actively participate in each assigned patient’s treatment plan
• Be the primary individual responsible for patient discharge and aftercare planning
