Fuller Hospital in South Attleboro, MA is seeking a Clinical Social Worker for a Full Time, 40-hour weekly position. You will provide comprehensive social work services in inpatient/partial hospital settings and coordinate discharge and aftercare planning with treatment teams.

Qualifications

• Master’s degree in social work, counseling, or related field.
• Licensed or eligible for LCSW/LICSW/LMFT/LHMC licensure.
• Hospital social work experience preferred.

Responsabilités

• Provide a comprehensive range of social work services to patients in inpatient/partial hospital settings.
• Act as primary contact with external providers and participate in each patient’s treatment plan.
• Lead patient discharge planning and aftercare coordination.
