Job summary

AdventHealth in Orlando, FL is seeking an experienced RN Case Manager to coordinate patient discharge planning and post-acute care. You will participate in multidisciplinary rounds, educate patients and families, assess readmission risks, and lead discharge planning conferences to ensure smooth transitions of care.

This role requires RN licensure with at least 2+ years in medical/hospital nursing, strong data analysis, and excellent communication across diverse populations.

Qualifications

• RN license required
• 2+ medical/hospital nursing experience
• BLS preferred; CCM/ACM preferred

Responsibilities

• Participates in multi-disciplinary rounds to review patient status, care progression, and discharge planning.
• Educates patients and families on illness impact and connects them with community resources.
• Assesses readmission reasons for patients and families.
• Organizes and leads discharge planning conferences with the care team.
• Documents discharge planning evaluations and barrier tracking for timely discharge.
• Assesses holistic discharge needs including social, financial, and functional factors.
• Reviews medical records to integrate clinical and social factors into the plan.
• Develops discharge plans with contingency steps for post-acute care.
• Uses standard workflows and technology to coordinate with post-acute services and ensure continuity of care.
• Performs other duties as assigned.
