Qualifications

• RN licensure required.
• 2+ years medical/hospital nursing experience.
• Care management/utilization management experience preferred.

Responsibilities

• Actively participates in multi-disciplinary rounds to review patient status, progress, and discharge plans.
• Educates patients and families about illness impacts and coordinates post-acute resources.
• Develops discharge plans with contingency plans for timely care coordination.
• Reviews medical records to inform discharge planning and transition of care.
