Job summary

Ascension St. Vincent in Indianapolis, IN, is seeking an experienced Registered Nurse for Inpatient Case Management. You will lead interdisciplinary care planning from admission through discharge, coordinating transitions home or to the next level of care.

Ideal candidates hold an nursing diploma and an active RN license; CCMC certification is preferred though ACM is acceptable. Acute care inpatient experience is highly valued, with benefits and professional development available.

Qualifications

• Requires RN licensure and nursing diploma
• CCMC certification preferred; ACM accepted if CCMC unavailable
• Acute care inpatient nursing experience is preferred

Responsibilities

• Lead the interdisciplinary care strategy from admission to discharge to ensure seamless and coordinated transition home or to next level of care.
• Advocate for patient needs by removing financial or logistical barriers to ensure resources and support for recovery.
• Evaluate medical necessity and care levels to ensure appropriate treatment while optimizing hospital and insurer resources.
• Collaborate with families, clinical teams and insurers to build a unified post-discharge plan and prevent gaps in care.
