Qualifications

• Minimum Education: Required: Bachelor's Degree
• Preferred Education: Master's Degree
• Minimum Experience: 3-5 years of previous job-related hospital experience
• Preferred Experience: 6-10 years of previous job-related hospital and Inpatient Care Management experience
• Preferred Certification: ACMA, CMGT-BC, CCM
• Recent Case Management experience is preferred.
• Recent clinical experience in medicine, surgery, critical care, telemetry, or ED is required.

Responsibilities

• Coordinate patient assessments for discharge planning with the multidisciplinary team.
• Facilitate daily rounds and address care coordination barriers.
• Educate patients and families regarding available options of care throughout the continuum.
• Advocate for resources and negotiate healthcare services to support safe outcomes.
