Job summary

UChicago Medicine in Chicago, IL is seeking an Adult Inpatient Case Manager (Registry) to partner with the clinical team and coordinate inpatient care from admission through discharge.

The role supports Care Coordination on the inpatient unit and requires a nursing license in Illinois, a BSN or ADN with BSN preferred, and 3+ years of nursing experience. Weekend shifts are typical with a 8:30am–5pm schedule at Hyde Park.

Qualifications

• Bachelor’s degree in nursing from an NLN accredited school of nursing.
• Current Illinois Registered Nurse license.
• 3+ years of nursing experience.
• ADN nurses must enroll in BSN within 6–9 months and complete BSN within 30 months; ADN nurses receive tuition reimbursement toward BSN.

Responsibilities

• Plans, monitors, and coordinates patient care using the plan of care or clinical pathway.
• Identifies patient and family needs for consultation with other multidisciplinary specialists and assures needs are met.
• Plans and conducts or participates in patient care conferences.
• Assesses patient discharge needs and ensures timely discharge related functions.
• Provides consultation to staff nurses in evaluating current patient status and plan for care.
• Ensures comprehensive information for the patient’s hospital course is documented in the chart.
• Performs other duties as assigned.
