Job summary

Advocate Health is seeking an experienced RN Case Manager inpatient at St. Luke’s in Milwaukee. You will assess patients, coordinate care with families and the care team, and plan discharges to ensure safe transitions and regulatory compliance.

Responsibilities include coordinating transfers, delivering CMS-compliant UM activities, and collaborating with Social Work to implement effective care plans. A current RN license and 2+ years of clinical nursing are required.

Qualifications

• Licensed RN with a current state license.
• Bachelor’s degree in Nursing preferred.
• Minimum 2 years of clinical nursing experience.

Responsibilities

• Assess patient needs and create care plans and interventions.
• Coordinate with patients, families and the multidisciplinary team to meet goals.
• Provide discharge planning and ensure regulatory compliance.
• Coordinate transfers and handoffs across units and settings.
• Deliver case management addressing access, finances, housing, and support services.
• Document discharge planning and UM activities accurately and on time.
• Collaborate with Social Work and care staff to implement the plan of care.
