Qualifications

• Bachelor’s Degree in Nursing or Nursing Diploma or 6 years of relevant experience in lieu of a degree.
• 3 years in a clinical nursing role.
• Current State of PA RN licensure OR current multi-state licensure through eNLC.
• Nationally recognized Case Management Certification within 5 years of start date.
• Critical thinking and problem solving.
• Flexibility and adaptability to change.
• Strong communication and collaboration skills with providers, peers, and patients.
• CPR certification.
• Act 34/33/73 background clearances; professional certification within 5 years.

Responsibilities

• Coordinate clinical care with patient/family and interdisciplinary team.
• Contribute to comprehensive patient/family assessment including social determinants and barriers.
• Identify barriers and analyze risk to plan safe transitions and care coordination.
• Advocate for patients through communication with providers, payers, and post-acute partners.
• Educate patients and families on care, documentation, and resource use.
• Pursue professional development through certifications, mentoring, and committees.
• Adhere to hospital policies and procedures.
