About This Role

The RN Case Manager Inpatient is responsible for creating, implementing, and evaluating care plans for patients and their families across the health continuum. This role is ideal for experienced nurses who excel in collaboration and patient advocacy.

• Schedule: Part-time, 48 hours every two weeks, Daylight M-F, rotating weekends and holidays

What You'll Do

• Coordinate clinical care with patients, families, providers, and interdisciplinary team members.


• Complete comprehensive assessments of patients and their families, including clinical needs and social determinants of health.


• Identify barriers and analyze situations for assigned patient populations to determine safe transition and care coordination plans.


• Advocate for patients and families by communicating effectively with providers and interdisciplinary team members.


• Provide education on industry standards, documentation requirements, and resource utilization to patients and families.


• Seek professional development opportunities through certification and participation in committees.


• Adhere to policies, procedures, and regulations of hospitals and clinical settings.

Requirements

Education:

• Minimum Bachelor’s Degree in Nursing -OR- Bachelor’s Degree and Nursing Diploma -OR- 6 years of relevant experience in lieu of a degree.

Experience:

• 3 years in a clinical nursing role.

Licenses & Certifications:

• Current State of PA RN licensure OR current multi-state licensure through the enhanced Nurse Licensure Compact (eNLC).


• Nationally recognized Case Management Certification within 5 years of start date.


• CPR certification required.


• Act 34, Act 33, and Act 73 clearances required.
