Job summary

The Case Management Department at Allegheny General Hospital is seeking a professional registered nurse to coordinate and evaluate patient care plans across the health continuum. The part-time Case Manager will collaborate with providers and care teams to optimize outcomes and reduce readmissions.

Responsibilities include comprehensive assessments, care coordination, and patient/family education, with a focus on transitioning care safely and efficiently in a fast-paced inpatient setting.

Qualifications

• RN licensure in PA or via eNLC.
• 3 years in a clinical nursing role.
• National Case Management Certification within 5 years of start date.
• CPR certification and background clearances.
• Strong communication and collaboration with patients, families and providers.

Responsibilities

• Coordinates clinical care with patient, family, providers and interdisciplinary team.
• Performs comprehensive assessments including social determinants of health.
• Identifies barriers and plans safe transitions and care coordination.
• Advocates for patients and families with providers, payers, and post-acute partners.
• Educates patients and families on care plans, resources, and discharge planning.
• Pursues professional development and participates in department/hospital activities.
• Adheres to hospital policies and regulatory requirements.
