Job summary

Qualifications

• RN license in the state where practice occurs.
• Bachelor's degree in nursing preferred; required in Fargo/South Dakota regions.
• Minimum two years' clinical experience preferred.
• BLS within 60 days of hire; CCM encouraged when eligible.

Responsibilities

• Provide leadership to ensure effective resource management for patient care.
• Collaborate with licensed practitioners and inter-professional teams for care coordination.
• Coordinate discharge planning and referrals to post-acute resources.
• Advocate for patients across the continuum of care and promote evidence-based practice.
