Job summary

Mercy Hospital Springfield is seeking an Inpatient Social Worker to join the Care Management team. The role involves assessing, planning, advocating, and coordinating patient care from admission through discharge to ensure safe transitions post hospitalization.

As part of Mercy’s interdisciplinary team, you will contribute to care management practices, utilizing performance metrics and best practices to enhance patient experience, safety, and quality of care. PRN Days with varied hours.

Qualifications

• Graduate of an accredited School of Nursing, required.
• Bachelor's Degree of Science in Nursing, minimum required.
• Current License in the state of employment, required.
• BLS (CPR) at hire date, required, or within 90 days of hire.
• Certification in Case Management, preferred.
• 2-3 years acute care hospital setting, preferred.
• Care Management or Utilization Management experience, preferred.

Responsibilities

• Assess, plan, advocate, and coordinate care from admission to discharge.
• Ensure safe transitions post hospitalization using performance metrics and best practices.
