Job summary

Mercy Hospital Springfield is seeking an inpatient Care Management professional to support patient-centered transitions across the hospital stay. The role collaborates with nurses, social workers, and care teams to plan discharge and coordinate post-acute needs.

Education required includes a Bachelor's or Master's in Social Work with licensure in Missouri. Experience in acute care is preferred, and the department focuses on safe, timely care transitions for patients and families.

Qualifications

• Education: Bachelor's or Master's in Social Work.
• Preferred Licensure: LMSW, LCSW, or LPN/RN in Missouri.
• Preferred Experience: Acute care hospital setting.

Responsibilities

• Care Management staff assess patients throughout hospitalization to identify medical, psychosocial, functional, financial, and environmental factors affecting treatment, recovery, or discharge.
• In collaboration with patients, families, providers, and community partners, develop individualized care transition plans aligning with patient goals and post-acute needs.
• Coordinate discharge planning, psychosocial assessment, education, advocacy, resource referrals, and post-acute services such as home health, rehab, SNF, hospice, or community supports.
