Job summary

Mercy is seeking a Care Manager to join the interdisciplinary team, overseeing care from admission through discharge and ensuring a safe transition post hospitalization. The role emphasizes advocacy, planning, and coordination in alignment with Mercy's mission and service standards.

Required education includes an MSW (or pending conferment) with LMSW/LCSW licensure in Missouri, 0-2 years in acute care, and BLS CPR at hire or within 90 days.

Qualifications

• MSW degree or equivalent for Social Workers hired after 2011, awaiting conferment.
• 0-2 years' experience in acute care hospital setting.
• Current LMSW or LCSW licensure in state of employment.
• BLS CPR at hire date or within 90 days of hire.
• Certification in Case Management is preferred.

Responsibilities

• Assess, plan, advocate, and coordinate care from admission to discharge to ensure a safe transition post hospitalization.
• Maintain patient experience, safety, and quality of care using performance metrics and best practices.
• Collaborate with an interdisciplinary team to coordinate care and discharge planning.
• Support transition planning and length of stay oversight.
