Qualifications

• Two+ years RN experience across Med/Surg, ICU, ED, Home Health, SNF, Hospice, LTAC, rehab.
• Successful completion of an RN program by date of hire.
• High School Diploma or GED required.

Responsibilities

• Coordinates post-discharge patient care needs to assure timely and effective discharge.
• Independently documents patient assessments that are thorough, timely, and age appropriate.
• Communicates with patients, families, and health care team to develop plans of care.
• Facilitates resolution of barriers to safe transfers using care conferences.
• Ensures ongoing contact with interdisciplinary staff and continuing care services for safe transitions.
• Ensures regulatory and compliance standards are met with the care team.
• Performs duties as requested.
