Job summary

SouthEast Alaska Regional Health Consortium in Sitka, Alaska, is seeking an RN Care Coordinator to ensure patients receive the right care at the right time during hospital stays and transitions home.

You will lead discharge planning, coordinate across the continuum, perform utilization reviews, collaborate with physicians, nurses, social services, and families to develop patient‑centered care plans, advocate for patients, and support case management.

Qualifications

• RN license required in Alaska. ADN or equivalent degree required.
• Experience coordinating care across hospital and community settings.
• Strong knowledge of nursing principles and patient safety.

Responsibilities

• Lead discharge planning efforts and coordinate safe transitions home.
• Perform utilization review to support appropriate levels of care.
• Collaborate with physicians, nursing staff, social services, patients, families, and external agencies.
• Identify barriers to discharge and coordinate solutions for better outcomes.
• Advocate for patients while ensuring regulatory and reimbursement compliance.
