Job summary

PeaceHealth Ketchikan Medical Center is seeking an experienced RN Care Coordinator for inpatient care management on a 0.60 FTE day shift. This role partners with physicians, nurses, social workers, patients, and families to coordinate care, advocate for patient needs, promote positive outcomes, and ensure appropriate utilization of healthcare resources.

The position is ideal for an experienced nurse who enjoys collaboration, care planning, and helping patients achieve the best possible outcomes.

Qualifications

• Graduate of an accredited School of Nursing.
• Current Registered Nurse (RN) license in the state of practice.
• Current Basic Life Support (BLS) certification.
• Minimum of three years of acute care clinical nursing experience.

Responsibilities

• Coordinate and oversee patient care plans throughout hospitalization and discharge planning.
• Collaborate with interdisciplinary teams to create individualized care strategies.
• Conduct comprehensive patient assessments, including clinical, psychosocial, environmental, and discharge needs.
• Advocate for patients and families while helping remove barriers to care.
• Ensure appropriate utilization of healthcare resources and levels of care.
• Facilitate communication between providers, patients, families, and community resources.
• Identify opportunities to improve patient outcomes, care transitions, and patient satisfaction.
• Support timely and effective discharge planning to promote successful recovery and continuity of care.
• Serve as a trusted resource and leader within the care management team.
