Medical Edge in Sitka, AK seeks an experienced RN Inpatient Care Coordinator to support patients during their inpatient stay by coordinating care plans, facilitating discharge planning, and ensuring continuity of care through collaboration with the interdisciplinary team.

The role combines strong clinical nursing skills with care coordination and patient advocacy to achieve optimal outcomes and a positive patient experience, while guiding families through treatment options and post-discharge

Qualifications

• Associate Degree in Nursing (BSN preferred).
• Active and unrestricted Registered Nurse license.
• Current BLS certification required.
• Strong clinical assessment, communication, and care coordination skills.
• Acute care, case management, utilization review, discharge planning, or care coordination experience preferred.
• Ability to work collaboratively within a multidisciplinary team.

Responsibilities

• Conduct comprehensive patient assessments and identify care coordination needs.
• Collaborate with physicians, nurses, therapists, social workers, and community resources.
• Develop and manage individualized care and discharge plans.
• Coordinate referrals, follow-up appointments, and post-discharge services.
• Educate patients and families regarding treatment plans and resources.
• Monitor patient progress and address barriers to care.
• Advocate for patients and assist with navigating the healthcare system.
• Maintain accurate documentation within the EMR.
• Participate in quality improvement and patient safety initiatives.
