Job summary

Advocate Health is seeking a Part-time Physical Therapist for the inpatient rehabilitation unit in Park Ridge, IL. The role involves evaluating patients, establishing care plans, and coordinating discharge with home programs and family teaching.

You will document care, monitor progress, and collaborate with the care team to meet patient goals. Requirements include a Doctorate in Physical Therapy and state licensure, with BLS.

Qualifications

• Doctorate in Physical Therapy from an accredited program.
• Licensed Physical Therapist in the state.
• BLS for Healthcare Providers (AHA) within 90 days.
• No experience required.
• Intermediate computer skills.
• Excellent communication, interpersonal and organizational skills.

Responsibilities

• Interpret physician referrals and complete musculoskeletal, neuromuscular, cardiopulmonary, and/or integumentary evaluations.
• Develop and implement a plan of care and adjust it based on outcomes.
• Plan discharge and provide home programs and family education.
• Document evaluations, progress notes, and discharge summaries per policy.
• Supervise staff and assist with coverage as needed.
• Promote patient safety and participate in safety and infection control measures.
