Job summary

Advocate Health is seeking a licensed Physical Therapist for our Outpatient Rehabilitation program in Illinois. This full-time role involves evaluating musculoskeletal, neuromuscular, cardiopulmonary, and integumentary conditions, and developing individualized plans of care.

The position requires strong clinical skills, documentation accuracy, and collaboration with physicians and the care team. Hours are 40 per week with 2 evenings until 7:00 p.m., and a competitive benefits package is offered.

Qualifications

• Doctorate in Physical Therapy from an accredited school.
• Licensed Physical Therapist in the state you work in.
• BLS for Healthcare Providers certification.
• Intermediate computer skills.
• Excellent communication and organizational abilities.

Responsibilities

• Interprets referrals and completes musculoskeletal, neuromuscular, cardiopulmonary, and/or integumentary evaluations.
• Establishes a plan of care and implements treatment based on evaluation results.
• Monitors outcomes and adjusts treatment plans; provides updates to physicians and team.
