Job summary

Texas NeuroRehab Center is seeking a compassionate Occupational Therapist for our new Sage Inpatient Rehabilitation Unit. You will perform comprehensive OT evaluations, develop individualized treatment plans, and deliver evidence-based interventions to improve ADLs, IADLs, mobility, cognition, and independence.

You will collaborate with an interdisciplinary team to optimize patient outcomes, contribute to discharge planning, and educate patients and families.

Qualifications

• Complete comprehensive occupational therapy evaluations and develop individualized treatment plans.
• Provide evidence-based interventions addressing ADLs, IADLs, cognitive function, upper extremity function, vision/perception, mobility, and community reintegration.
• Assess and recommend adaptive equipment and environmental modifications.
• Monitor progress and modify treatment plans to achieve functional goals.
• Educate patients, families, and caregivers on diagnosis, treatment strategies, safety, and discharge recommendations.
• Participate in discharge planning and post-acute care planning.

Responsibilities

• Collaborate with physicians, nurses, therapists, case managers and other professionals.
• Participate in interdisciplinary team meetings and family education sessions.
• Contribute to development of rehab goals and plans of care.
