Job summary

Responsibilities

• Collects and analyzes data, develops and implements an individualized plan, coordinates care delivery and evaluates progress toward outcomes.
• Directs care and utilizes assistants appropriately in delivering direct patient care.
• Collaborates with patient, family, physicians and staff; documents plan and communications.
• Participates in governance, committees and unit projects.
• Evaluates patient responses to nursing interventions and updates the care plan.
• Uses evidence-based practice to guide decisions.
