Job summary

UChicago Medicine in Chicago's Hyde Park is seeking a Physical Therapist II for inpatient services. This full-time day shift role provides comprehensive assessments, develops therapy plans, and collaborates with the care team to optimize patient outcomes.

You will mentor students and less experienced therapists, document progress, and participate in quality improvement initiatives. Candidates should have at least two years of PT experience, Illinois license, and APTA CI credentialing; CPR/BLS is

Qualifications

• Requires graduation from an approved physical therapy school.
• Minimum of 2 years’ clinical physical therapy experience.
• Declaration of intent to develop advanced, clinical knowledge and skills in a focused area of clinical practice.
• Demonstrated sustained active engagement in performance and quality improvement initiatives and/or program development.
• Completion of APTA CI Credentialing Course.
• Requires current physical therapy license in the State of Illinois (or eligibility to practice) and CPR/BLS certification.

Responsibilities

• Conducts comprehensive evaluations to determine physical therapy problems, functional levels, and treatment plans.
• Collaborates with the interdisciplinary team to establish patient disposition.
• Develops physical therapy interventions in collaboration with patient/family and treatment team goals.
• Performs appropriate physical therapy techniques and procedures.
• Ensures continuity of care through timely and effective communication with team members.
• Attends rounds and clinics, providing patient updates and discharge recommendations.
• Identifies and recommends necessary patient care equipment prior to discharge.
• Documents evaluations, progress notes, and procedures within established timeframes.
• Educates patients and families in home exercise programs.
