Responsibilities

• Completes daily notes reflecting patient response to treatment and status for discharge.

Education

Bachelor’s Degree in Physical Therapy

Licensed Physical Therapist in State of Illinois

BLS from American Heart Association

Masters or Doctoral Degree in Physical Therapy
