Qualifications

• Bachelor’s Degree in Physical Therapy.
• Licensed Physical Therapist in State of Illinois.
• BLS from American Heart Association upon hire.
• Masters or Doctoral Degree in Physical Therapy (preferred).

Responsibilities

• Direct patient care, teaching, and participation on committees.
• May serve as clinical instructor and mentor.
• Ensure patient plans of care and documentation in EMR.
