• Provides direct patient care and participates in teaching and committees.
• Maintains and documents patient plans of care, collaborating with PT/PTA(s).
• Communicates effectively with patients, families, and healthcare teams; uses EMR for documentation.
• Supports continuous improvement and cost-efficiency initiatives within the department.

Education

Bachelor’s Degree in Physical Therapy

Licensed Physical Therapist in State of Illinois

Masters or Doctoral Degree in Physical Therapy

BLS Certification (American Heart Association upon hire)
