Responsibilities

• Performs a thorough and timely assessment of patient’s condition including physical, psychological, social and rehabilitative aspects.
• Intervenes to address problems and needs; plans care according to standards; manages time and priorities.
• Evaluates care effectiveness and revises plans as needed.
• Documents accurately in patient records and communicates with the care team.
• Maintains safety standards and coordinates with Case Manager for discharge planning.
